I would cover all the demographics: age, gender, socio-economic background, race.
The rest would depend on how you're measuring intelligence, whether it be by acomplishments, IQ, etc.....
Or you can go another route, like this:
How do you define human intelligence?
How significant do you believe standardized tests, such as IQ tests, are in measuring intelligence?
How do you believe human intelligence should be measured?
Do you believe environmental factors can influence a person's intelligence? If so, what factors?
What are some good questions for a survey on "human intelligence?
When will we see any?
