Were any caused by road debris?
Were any caused by road conditions such as ice, rain, etc.?
Were any caused by mechanical error?
If so, which vehicle...yours or other drivers?
Were any caused by driver error?
Who was at fault if it was driver error?
Was there a ticket issued by an officer?
Were there any bodily injuries to any of the occupants?
Was there any additional property damage other than to the
vehicles?
Were there any other contributing factors such as drugs, or
alcohol, exhaustion due to excessive driving, etc.?

Depending on what results you want to achieve will determine what sort of questions you would want to ask.
The first question that you should be asking... Have you ever been involved in a vehicle accident? I do know some people that have been driving for 40 years, and have not had an accident.
What sort of accident were you involved in? This can be described as nose to tail, chain collision, side swipe, T boned, lose control and or run off road. That is a few of the examples that I can come up with... I am sure that you can think of more...
What were the road and weather conditions like at the time of the accident? This can have a bearing on the accident happening in the first place.
What was the condition of the vehicle that you were in at the time of the accident? Was the vehicle roadworthy, or did it have any faults that the driver knew about? This can also have a bearing on the accident happening in the first place.
At the time of the accident, was the driver under the influence of alcohol or any (legal or otherwise) drugs?
At what time of the day did the accident happen, and on what day of the week?
Were seatbelts fitted to the vehicle and were the seatbelts worn at the time of the accident?
Were there any other safety devices / equipment in the vehicle that was used / deployed in the accident? (Air bags, crumple zones.)
With regard to the accident, what could have prevented the accident from happening?
