How to create a table in access for a survey that has 10 questions and 5 possible answers.?

Answers are associated with a number i.e: 1-worst, 5-best. Later I would need to generate a report that would sum the answers.

How to create a table in access for a survey that has 10 questions and 5 possible answers.?
Each question should be one row and each possibly answer should be one column. Therefore, you'd end up with 10 rows and 6 columns (the question and each of the five possible choices). I would then mark the selected choice in bold or show them in a separate chart with the sum at the bottom.
