ok so i am doing a survey on depression for an assessment and i need some help with what else to ask in the survey..


i have already got :


*do you suffer from a depression illness?


*how old are you?


*how long have you hade depression for?


*what kind of treatment are you receving?


*does depression run in your family?


*is the treatment working for you?


those are the questions that i have got. these questions carn't be to personal and they have to be questions that will have YES or NO answers thanx heaps for your time..

What about asking them if they have someone to talk to or help them thru the hard time that they are going thru?
